Gary Formanek, 27

Gary "Sonny Boy" Alois Formanek, 27, of Tama died Tuesday, June 30, 2009, at his parents home in rural Toledo. Funeral Services will be held Friday, July 3, at 10:30 a.m. at Kruse-Phillips Funeral Home in Tama. Interment will follow at National Cemetery in rural Vining. Visitation will be held Thursday, July 2, from 5 to 7 p.m. at Kruse-Phillips Funeral Home in Tama.

Survivors include his companion, Tiara McConnell of Tama; his children, Liberty Sunshine and Evelynn May Formanek; his parents, Gary Formanek and Linda Shelltrack of Toledo; three sisters, Mikki (Dane) Potter of Marshalltown, Bonita Waseskuk (Allen Papakee) of Toledo; Antoinette Marie Waseskuk of Marshalltown; and several aunts and uncles.
